Draft notice for review — 8 October 2026. This document describes IASERIA's proposed general enquiry practices. Some operational details require confirmation before the online form is made publicly available.

Do not submit health information through the general enquiry form. Please do not enter diagnoses, symptoms, medical records, test results, prescriptions, photographs or other sensitive health information. This form is only for a basic contact request. If you need urgent medical help, contact local emergency services rather than IASERIA.

1. Who we are

IASERIA is an independent UK-based patient facilitation service helping people explore healthcare provider options in Kerala and elsewhere in India. IASERIA does not diagnose conditions, prescribe treatment, provide clinical care or guarantee results. For privacy enquiries, contact contact@iaseria.com.

To be confirmed before publication: the full legal name and postal contact address of the person or organisation responsible for processing personal information, and any applicable registration details.

2. Information collected

When you submit a general enquiry, we may collect your name, email address, optional telephone number, the time of your submission and technical information needed to operate and protect the form. Please do not provide medical information through this form.

3. Why we use it

We use these details to respond to your request, arrange a follow-up conversation if requested, manage correspondence and protect the website from misuse. We do not use general enquiry information for unrelated marketing without an appropriate basis and any required permission.

4. Legal basis

For a requested response, IASERIA expects to rely on legitimate interests or steps requested before entering into a service arrangement, depending on the circumstances. The applicable lawful basis must be confirmed in the final privacy notice. The enquiry checkbox acknowledges the notice and confirms that you want a response; it is not a substitute for a lawful basis or for any separate explicit consent required for health data.

5. Service providers and email

General website enquiries are submitted using Web3Forms, which delivers form submissions to IASERIA's email address. Email routing and mailbox services may also process contact information. Please see the providers' published information. IASERIA must verify the relevant processor contracts, hosting locations, international transfers and safeguards before enabling this integration for public use.

6. International coordination

If you later ask us to connect you with a hospital or specialist in India, we will explain what information is proposed to be shared, with whom and for what purpose before any transfer. Health information is special-category personal data under UK GDPR and requires additional legal and security safeguards. It should not be sent through the general enquiry form.

7. How long information is kept

IASERIA aims to retain general enquiry submissions only as long as necessary. The current Web3Forms dashboard setting is seven days for stored submissions; copies delivered by email may remain longer and require a separate documented deletion schedule. A final retention policy must be established and applied across all systems.

8. Security

We aim to limit access to enquiry information and use appropriate technical and organisational safeguards. No internet service is risk-free, and this form must not be used to send confidential clinical documents.

9. Your rights

Depending on the circumstances, you may have rights to access, correct, erase or restrict the use of your personal information, to object to certain processing and to request data portability. Contact contact@iaseria.com to make a request. You may also raise a concern with the UK Information Commissioner's Office (ICO).

10. Patient safety and independent advice

IASERIA offers non-clinical research, communication and coordination assistance. Hospital and specialist providers are responsible for clinical assessment, informed consent, treatment, outcomes and clinical aftercare. Treatment suitability, travel fitness, risks, costs, insurance and follow-up arrangements should be discussed directly with appropriately qualified professionals. IASERIA cannot guarantee appointment dates, prices, clinical outcomes or recovery times.
